This Privacy Policy explains what information Sworn Medical PLLC (“Sworn Medical,” “we,” “us”) collects through this website (swornmedical.com) and how we use it.

Important. This website does not collect protected health information (PHI). The website intake form is intentionally limited to non-medical contact information. Medical history, diagnoses, and records are handled separately through our HIPAA-compliant intake process after your initial consultation. Sworn Medical’s clinical practice complies with HIPAA when handling PHI; this policy covers only the website.

1. What we collect through this website

1a. Information you provide

When you submit the consultation request form, we collect:

  • Full name
  • Email address
  • Phone number (optional)
  • Branch of military service

We deliberately do not collect medical conditions, diagnoses, or other protected health information through this form. If you accidentally include such information, please understand it will be received as plain email and we recommend resending without it.

1b. Information collected automatically

Our hosting provider, Cloudflare, may automatically log standard technical information when you visit any page on this website, including:

  • IP address (often truncated or anonymized)
  • Browser type and version
  • Pages visited and timestamps
  • Referring website

We do not run third-party analytics scripts (Google Analytics, Meta pixel, etc.) on this site. We do not use tracking cookies. The Cloudflare logs are retained by Cloudflare under their privacy policy and are used by us only in the aggregate for operational and security purposes.

2. How we use your information

Contact information submitted through the intake form is used only to:

  • Schedule and conduct the consultation you requested
  • Follow up about your inquiry
  • Send service updates relevant to your request

We do not sell, rent, or share your contact information with third parties for marketing purposes. We do not add you to a marketing list.

3. Third-party service providers

To operate this website and deliver email, we use the following service providers:

  • Cloudflare — web hosting (Cloudflare Pages), DNS, content delivery, edge security, and inbound email routing
  • Resend — transactional email delivery (used to forward intake-form submissions to our practice email)
  • Google (Gmail) — receipt of intake-form notifications at our staffed inbox

These providers process limited data on our behalf. None of them receives protected health information through the website intake flow, because the form does not collect PHI.

4. Data security

All connections to swornmedical.com are encrypted in transit using HTTPS. The intake form is submitted over HTTPS and the resulting email is delivered over encrypted SMTP connections. We do not store form submissions in any database on this website — submissions are emailed and then deleted from any transient processing state by our hosting provider.

No system is perfectly secure. While we apply reasonable safeguards, we cannot guarantee absolute security of information transmitted over the internet.

5. Data retention

Intake-form emails are retained in our practice inbox for as long as needed to respond to your inquiry, schedule services, or maintain a record of business communications. You may request deletion of your contact information at any time by emailing [email protected].

Note: clinical records and PHI received through our HIPAA-compliant intake process (post-consultation) are governed by separate retention rules under HIPAA and applicable state law, not by this Privacy Policy.

6. Your rights

You may:

  • Request a copy of the personal information we hold about you that was submitted through this website
  • Request correction of inaccurate information
  • Request deletion of your contact information
  • Opt out of any communications from us

Residents of jurisdictions with specific privacy laws (California, Virginia, Colorado, the EU, etc.) may have additional rights under their applicable law. We will honor those rights on request.

7. Children’s privacy

This website is intended for adult veterans and their authorized representatives. We do not knowingly collect information from children under 18. If you believe we have inadvertently collected such information, please contact us and we will delete it.

8. Changes to this Privacy Policy

We may update this Privacy Policy from time to time. The “Effective” date at the top of this page indicates when the current version took effect. Material changes will be highlighted on the website.
